Question about the FE application

I just passed the FE exam in Louisiana. Therefore, my next step is to submit EI Certificate application to my states surveying board which in this case would be the Louisiana Professional Engineering and Land Surveying Board (LAPELS). 

When I was in college, I worked as a server and throughout my 5 years of serving I was caught off guard one time. A group of "nurses" came in. One old and the other a little younger. I worked at a family orientated restaurant so we didn't get many teenagers to young adults who were in that age range where you need to ID. I was the closing server and we had a rush so I had a million things on my mind. Long store short, I ended up accidentally serving an underaged girl a beer. I had to pay a fine of 500 dollars and as far as I am aware it was a warning as I got to keep my vendors license. However, this is probably still a "citation/violation" that needs to be reported on the application. 

So my question becomes, do I need to submit this in my application? If so, how should i present it? And most importantly, how will this affect my chances of not only becoming an EIT, but an actual PE?
