I'm going to be the spoil sport and suggest that the contractor didn't have the right saw so he just cored it out. Or the coring guy was there and they just had him do it.
More importantly, where is the rebar?

I like it!

There must be quite a few of those out there, I would imagine. Of course, around here we have the opposite problem. I have a lawyer friend (oxymoron?) who was having his dream house built up on top of the island and his contractor lost the plans while working on the foundation. They got all the way up to the roof on the second floor (all reinforced concrete building) before my friend started wondering why things were looking different than he thought they should. The contractor said he was too embarassed to ask for a second set of plans, so he was just "building by memory!"

Unfortunately there's no good, single picture that can capture THAT kind of f-up.
