I actually kept throwing up a red flag in a manufacturing/control system integration issue a few years back at my old company. No one would listen to me- no one in our engineering dept, two outside consutants, the engineer of the system we were integrating into our product. $20,000 or so later in damage to the control product they were asking me to help in gathering evidence that it wasn't our fault- pleading our case to the project owner. I just told my boss that I couldn't be a part of it since I had created a paper trail of my raising all the questions when no one cared to listen.
That kind of thing is frustraiting. At least it was only money (not lives) for us though.
