No. You just need to send the "Notification of Successful Exam" form. Are you registered for the state specific exams? Have you taken them? You may need to submit the re-examination form for those.

It honestly depends on what questions you get. I took it in April and passed it and didn't open the CBC/IBC material at all and only needed ASCE 7-10 and Hiner's workbook. That doesn't mean that you don't want to have it available though as you may get questions that you'll need the CBC/IBC to...

Correct. The form you submitted to get approved for seismic and surveying was the application for licensure. No additional forms are needed once you pass seismic and surveying. They usually update the license lookup the day after they send out results and your license number should already be...

You need to fill out the re-examination form and send it to the board. Once you get approved, you can take the exam in the quarter following the one in which you were approved. It's $150 per exam as far as fees go + prometric fees.

I can confirm. Failed surveying and only had one attachment. Oh well. Passed the 8-hour and seismic on the first try. Can't win them all right?

The goal of the board is to get results back on a monthly basis. So if you took an exam in April, the goal is for you to have a result mid-May. If you took your exam in May, you should have your result mid-June. The 30-day time frame doesn't really apply anymore.

Their goal is mid-May. However, in another thread we've discussed that it might be more likely the week of 5/21 since it's the first time they're doing testing/results on a monthly basis.
