    If I read you correctly, the second question was left unanswered. If that is the case, it will likely be scored "unacceptable". From what I have seen, one "unacceptable" is enough to knock you out completely. Not trying to be a downer, but the odds are certainly tilted against you.

    The state's approval may be completely unnecessary, since you already have a PE license. Check with NCEES. You may need to just register with them, pay the fee, and sit for the exam. You may have shot yourself in the foot by even asking permission.

    This question is best asked of the engineering license board. I have found many states boards to be quite helpful in obtaining the correct information. As much as you want your application to be correct, they want it to be correct more since it makes their job easier.
