Does anyone have a standard as-built certification letter that they use when they submit as-builts to city, state or DOT permit agency to certified the job complete?

 
Here in FL the different water management districts have a form letter that is signed and sealed to certify the stormwater management systems were built in accordance to the to plans and stormwater permit. There are also similar form letters through the FDEP to certify potable water and sanitary systems. Maybe some of those will be able to assist you in developing your own, hope that helps.

 
Most of the time it depends on the type of project and what aspects you are certifying. The larger authorities (state and federal) have standard forms available online for you to fill out/stamp while local authories (cities and towns) require a more project specific letter which you would create yourself.
