PE exam receipt/reimbursement

Does anyone know how to get a receipt from NCEES or Continental Testing (Illinois) for the cost of the test? Now that I've passed, my employer will reimburse me for the cost, but I'm not sure which entity I paid, and whether they emailed me a receipt, or how I could get access to another copy. Any ideas?

I took the PE in Illinois and ultimately paid Continental Testing. After I filled out the online application continental sent out an email that said the following

Continental emailed me a link to a pre-filled Illinois government worksheet that I had to mail in. This worksheet shows the amount that I paid to Continental testing, so I printed out this and used it to apply for reimbursement from my employer. Maybe try and find that old email from Continental with the same link?
