I've been hunting for the correct verbage used on most electrical title sheets in North Carolina, under the Appendix B section. I've seen it on numerous jobs in the past but can't seem to find a copy.

It's basically a designers statement that the engineer has to sign and date (underneath the wattage used vs allowed, etc).

Anyone have a copy of this?
