Why is the answer 63 kips (ASD) / 94 (LRFD) and not 125 kips (ASD) / 187 (LRFD)???

When to use the maximum uniform load given by the table and when to use the Vn/ factor of safety (the value given at the bottom of the column on pg. 3-74 / AISC manual)???

Don't have the question in front of me, but I'm pretty sure that shear failure governs in that case. Max shear is 94 kips

 
If the load were fixed at the center, then you would multiply the allowable shear load by 2. But this is a MOVING load, so if the load were placed nearest to one of the ends, then shear governs (62.5 k) as the allowable shear force would equal the applied load.
