Add the index to your cart, and go through the checkout process. You should not be charged any money (it shouldn't even ask you for any payment information), as the cost is listed as $0. You will need to have a PPI account, so if you do not already have one, go ahead and create one. Once you have completed the order, you will be emailed an order confirmation, from which you can print the index.
